About Kristina Loftis

Kristina Loftis, ASHM, LPN
Founder & Director of Clinical Operations
“Alabama clinics don't need a national call center - they need someone local who picks up the phone and actually knows their patients.”
Kristina Loftis is a licensed practical nurse and the founder of Partners in Care. She personally leads clinical operations - when your clinic calls, you're talking to the person who builds and runs the program, not a call center reading from a script.
Partners in Care is based in Alabama and works with Alabama clinics. That matters for things a national vendor tends to miss: how Alabama Medicaid handles remote monitoring, what a small clinic's front desk can realistically take on, and how to reach patients in areas where home internet isn't a given.
Who we work with
Partners in Care is sized for independent and small-to-mid-size practices - clinics that want a structured chronic care program without hiring and training an entirely new in-house team. That includes:
- Independent primary care practices
- Rural Health Clinics (RHCs)
- Federally Qualified Health Centers (FQHCs)
- Cardiology practices
- PT / OT clinics
